FOR SELLING PERSONAL CHOMETZ
My signature below acts as testimony, equivalent to one hundred witnesses that I the undersigned, have appointed and empowered Rabbi Menachem Mendel Gansburg to act in my stead that he should have the full power and authority to sell (or to appoint someone to sell in his stead) in an absolute sale with no conditions, to whomever he wishes to sell, and whichever form he wishes to sell, all Chametz that I have, wherever it may be, whether it is visible or mixed, or even if it is doubtful whether it is Chametz or not, in any form, whether food or drink, whether it is for human consumption or animal consumption, and all alcoholic drinks; and also to sell all animals, birds and fish which I am responsible for and they consume Chametz; and also to sell all Chametz Keilim (utensils and vessels), and Keilim which have Chametz on them; whether they are in my possession now or they are in transit and will arrive before the time of Chametz prohibition, even if I am unaware of them; and also to lease all the places where the Chametz and/or Keilim are placed, and/or where the animals, birds and fish are located; for all of the above I empower Rabbi Menachem Mendel Gansburg and give him the power to sell and lease to whomever he sees fit, in any form he sees fit, whether he himself or him appointing an agent to his liking, in the best possible form of sale according to the Torah and the law of the land, on Wednesday, the 14th of Nisan of this year 5786.
